Once you receive your Foam Impression Kit, you are only 3 steps away from relieving foot pain.

1. Take the impressions of your feet by following the instructions in the "Recommended Procedure for Taking Foam Impressions". This impression of your feet will serve as the foundation of your new pair of custom made orthotics.
2. Complete the "Orthotic Order Form". This form must be completely filled out so that our laboratory has all of the information required to make your new custom made orthotics. Please indicate which type of orthotic you want.
3. Enclose the order form in the foam impression kit and clearly write your name and address in the upper left hand corner of the carton. Adhere the US Post Office pre-paid mailing label to your box. Apply a strip of tape to the seam of the box to seal the carton.

In a few weeks, your new pair of custom-made orthotics will be shipped directly to your home. Included in the package with your new orthotics you will receive specially designed wearing instructions and information regarding discounted additional pair orders along with additional product and service offerings.
